I am a UK taxpayer and I want PHAKAMISA UK to treat all my donations to PHAKAMISA as Gift Aid donations until notified otherwise. I presently pay income tax and/or capital gains tax, that is at least equal to the total tax reclaimed on my Gift Aided donations and in completing and submitting this form I am agreeing to this declaration.
